Filling out these forms before your visit saves time at check-in. All forms can be completed online. If you'd rather do them in person, we'll provide paper copies at the front desk.

  Required before evaluation and treatment.
• Medical History Update
  For returning patients with new conditions or medications.
• Release of Records
  To request medical records be sent to another provider.
• Sliding-Scale Application
  If applying for reduced rates below the standard self-pay rate.
